The goal of the Pennsylvania Elks Home Service Program is to help improve the quality of life for children and adults with disabilities and for them to become as independent as possible. The Elks Home Service Nurse provides in home non-traditional case management to help prevent medical complications and to promote optimal health. The Elks Home Service Nurse program provides a Registered Nurse or Medical Care Coordinator to visit children and adults with developmental disabilities in their homes, free of charge.
While the Elks Home Service Nurse does not provide medical care, they can make home visits to assist with all of the above listed services, attend medical appointments in an effort to assist the family with understanding medical terminology, treatments, and specific training to assist with follow through of medical advice.
The primary role of the Elks Home Service Nurse is to assist an individual with a disability and/or families connect to resources. An individual of any age who has a developmental disability that is present before 22 years of age is eligible to receive services, which can be continued throughout a person's lifetime if needed. In many cases, services provided to children and their families helps to ease the transition into adulthood.
