There are two services offered through the Attendant Care Program of UCP/CLASS. These are:
The Attendant Care Program coordinates in-home assistance for adults ages 18-59, who have physical disabilities and are able to supervise attendants. Services include personal care and light housekeeping. Consumers may hire their own attendants or have them provided by UCP/CLASS or a home health agency.
Assistance is available for Activities of Daily Living (ADLs). Through this program, individuals are given the opportunity to stay comfortably at home and receive help with tasks such as personal care, meal preparation, laundry, shopping, and light housekeeping.
A Service Coordinator assists individuals who request attendant care services with the application process. Once the individual is participating in the program, the Service Coordinator then monitors and coordinates attendant care and other services. Approved hours and specific services are based on each individual's needs and available supports.
Attendant Care is not appropriate for individuals who:
The Nursing Home Transition program is a service offered to individuals who are currently residing in nursing homes and wish to return to their community to live in their apartment or house. This program is for individuals ages 18-59.
Funding may be available to qualifying individuals to help with one time expenses involved in moving into a home or apartment. These might include such things as home modifications for accessibility, security deposits, moving expenses, essential furnishings, etc. In-home services can then be offered to provide ongoing personal assistance to allow individuals to lead the most independent life as possible.
The services you might be eligible to receive will depend on your needs and abilities as well as you income and asset levels.
